What You Need to Know When
Enrolling a New Dependent

New Born Child

A new born child is eligible for coverage on the date of his or her birth. The fastest way to add them to your coverage is to submit the Dependent Enrollment Form and include the Hospital Birth Announcement. The child's full name, date of birth and Member's name must be stated on the Announcement.

Once you receive the child's Birth Certificate and Social Security Card, please send them to the Fund Office by using the Secure Upload Center.

Stepchild or Adoption

If you would like to enroll a stepchild from a marriage or have adopted a child, please submit the Dependent Enrollment Form. You will need to make sure you have the child's Birth Certificate and Social Security Card to upload during the enrollment process.

Marriage

If you would to enroll a spouse, please submit the Dependent Enrollment Form. You will need to make sure you have the spouse's Birth Certificate, Social Security Card, and the Marriage Certificate.

If your spouse has kept their maiden name, please download and complete the Maiden Name Form. You can return it to the Fund Office by using the Secure Upload Center or email it to enrollment@carpenters.fund.

Please include copies of all Health Insurance Cards that any Eligible Family Member is enrolled in besides EAS Carpenters Health Fund Coverage. You will also need to indicate the effective date and which benefits are covered such as; Medical, Dental, Vision or Prescription.

Medical and Vision Card

This is for your major medical insurance and vision benefit. You should receive your cards about 2 weeks after you become eligible for benefits. Use it when visiting doctors, hospitals, or specialists.

Prescription Card

Prescription coverage is through Express Scripts. You should receive your cards about 2 weeks after you become eligible for benefits. Use it at the pharmacy for all prescriptions.

Cigna Dental Card

Dental coverage is through Cigna Dental. You and your eligible dependents will receive this cards about 3 to 4 weeks after you become eligible for benefits.

Health Reimbursement Card (HRA)

If you are an HRA-eligible Participant, you will receive two (2) HRA Payment Cards at your home address that you and your family can use for point-of-sale eligible expenses.
