Coordination of Benefits Form
The Annual Enrollment/COB Form is sent every year to all Eligible Participants to ensure that the Fund has the most accurate information regarding your census, dependent/family information, and other coverage (employer sponsored, state sponsored) coverage to coordinate claims. The Fund is in the process of updating its website and the “My Profile” page which usually updates the COB each year. As a result, the COB is delayed. Please note, nothing will change with your benefits or vacation check as a result of the COB being delayed. Thank you for your understanding as we make upgrades.
