Reach Out to Nurse Fran!
Through the Fund’s partnership with Temple Health, the Complex Care Coordinator, Nurse Fran, has been a welcomed addition to the Fund staff.
She can assist Participants with:
Post-hospital stay guidance.
Medication review and education.
Preventative care management.
Chronic disease follow-up care and guidance.
